Offices: 7 offices, Dental room, 2 exam rooms, Boardroom/kitchen area and supply room.

Number of Health Staff: Community Health Nurse, NNADAP, Community Wellness Coordinator, Home Care, Nurse/Aide, Medical Transportation, Dental /Dental aide/Dental Therapist/ and COHI, Maternal Child Health, Physician Services, Nurse Practitioner, Receptionist, Janitorial/ Maintenance and Psychiatrist.
